    Dear Sir, 

    My family consists of ........... members none of whom has any Ration Card. However, I give below details of every family member. Kindly issue (them) necessary Ration Cards in their names and oblige.

    In this connection, I may mention here that previously I had been living at/in........ with my family, and I returned our Ration Cards to the proper authority before leaving that place. (The receipt of surrender may be shown/submitted to you as and when necessary). I have shifted here with my family only on....... and have been residing at the above address since then.
