    Out of six Ration Cards held by my family, two are for the children. Ration Card No........... in the name of my first son Mr.............. now requires a change as he has attained the age of 8 years on 30.09.1994.

    The concerned Ration Card (No.) is being sent herewith for kindly issuing an Adult Ration Card in place of the same.
